  2. Print & fill out the Intake forms

  3. Upload the signed and completed forms by clicking "upload intake forms"

  4. If using insurance, upload copies of your insurance card and drivers license by clicking "member ID & drivers license". 

  5. If you can't upload files, please send them to info@ekpsych.com
